Dear IDHS Stakeholders,
As you may know, the Illinois Department of Human Services (IDHS), working closely with our sister State agencies, is responsible for administering the Williams and Colbert Consent Decrees programming in Illinois. IDHS is issuing a Request for Application (RFA) to award grants to eligible organizations to serve as grantees for the Comprehensive Class Member Transition Program, using their expertise and experience in executing and managing transition services (including, but not limited to, Outreach, Resident Review/Evaluation, and Transition Coordination) for class members as direct providers and/or through subrecipients. To optimize coordination, the state has been divided into 7 regions, and at least one grantee organization will be selected for each designated region. If you wish to be considered as a subrecipient or to coordinate an application with other providers, please complete this Survey for Potential Subrecipients in order to have your information shared with and available to other interested organizations. Signing up as a potential subrecipient does not preclude an organization from applying to be an overall grantee.
All applicants interested in serving as a grantee can find the Request for Application posted on this site by close of business on Monday, October 21, 2019. All applicants interested in serving as a grantee must submit their application by Friday, December 20, 2019 at 5:00 pm CT. IDHS will host a live webinar on Wednesday 10/30/19 from 10:00am-12:00pm CT. The link to the webinar will be posted to this site. Potential applicants and subrecipients will also have the opportunity to ask questions, both at the webinar and through email.
