The Illinois Prescription Monitoring Program (PMP) is part of the Illinois Department of Human Services.

The Illinois PMP does not guarantee any report to be entirely accurate. The records contained in the PMP are reported by the dispensing pharmacies and mistakes, while rare, are possible. PMP reports should only be used to supplement a patient evaluation, to confirm a patient's drug history, or to document compliance with a therapeutic regimen. What is the PMP?
The Illinois Prescription Monitoring Program (PMP) collects information on controlled substance prescriptions dispensed in Illinois. The PMP web site allows prescribers and dispensers to view a current or prospective patient's prescription history.
The goal of the PMP is to assist prescribers and dispensers in the effective treatment of patients seeking medical care.
Voluntary and Free
The program is voluntary, free, and is intended only to assist medical providers to better serve their patients.
Easy to Use
It is minimally disruptive to healthcare providers and there is no barrier to patient therapies.
What information does the PMP provide?
- Schedules II-V controlled substance prescription information.
- Records viewable for 12 months and kept on record for two years.

- Retail pharmacies dispensing in Illinois report on a daily basis.

To sign up for a free account, visit www.ilpmp.org. On the home page, you will find a Pharmacist Registration link and a Prescriber Registration link.
Law Enforcement and Patients can indirectly access information from the PMP. In order to do so, forms are available on the home page www.ilpmp.org.
