PMPnow Illinois Prescription Monitoring Program - DHS 4198

State of Illinois
Department of Human Services

The Illinois Prescription Monitoring Program (PMP) is housed by the Illinois Department of Human Services.

The Illinois PMP does not guarantee the complete accuracy of a patient's controlled substance prescription history. The records contained in the PMP are reported by the dispensing pharmacies and are therefore dependent on the accuracy of the information provided. PMP reports should only be used to supplement a patient evaluation, to confirm a patient's drug history, or to document compliance with a therapeutic regimen.  If you notice an error in reporting please email us at

What is the PMP?

The Illinois Prescription Monitoring Program (PMP) collects information on controlled substance prescriptions dispensed in Illinois.  The PMP web site allows prescribers and dispensers to view a current or prospective patient's prescription history.
