15.1 Transportation for Parents
Parents of infants and toddlers within the EI Program may enroll to be reimbursed for mileage reimbursement incurred as a result of transporting their child to/from EI services identified on their child's IFSP. Those parents interested may enroll as transportation providers to provide this service to their child only.
15.2 Enrollment to Bill
The application for enrolling as a provider of transportation may be found on the Provider Connections website at www.wiu.edu/providerconnections under Enrollment. For questions, the parent may call Provider Connections directly at 1-800-701-0995. As part of the enrollment process, the parent will be asked to supply proof of current vehicle insurance, a valid driver's license and up-to-date vehicle registration.
15.3 Authorization and Billing
Once enrolled, the parent will need an authorization prior to billing.
For billing, the parent should complete a Transportation Billing Form which may be located on the Provider's section under Downloads/Forms on the EI CBO Website.
As of April 2015, the current HFS mileage rate for non-emergency vehicles is 23 cents per mile and will reimbursed for loaded miles only.
The parent will use procedure code A0090. Departure/Destination Codes are R for Residence or D for Medical Services.
