Dear Community Providers:
On December 7, 2016, the Division of Mental Health sent the following message to contracted providers of mental health services to children and adolescents:
Chapin Hall at the University of Chicago, in collaboration with the Illinois Department of Human Services Child and Adolescent Division with support from the McCormick Foundation, is conducting a survey to inform state and local public agencies as well as foundation funders about current practices of nonprofit agencies. Specifically, we are focusing on how organizations define and report the outcomes of outpatient mental health services provided to school age youth (ages 5-21). Through this survey we seek to understand the types of clinical and service data that are useful in describing the progress of clients as well as the data that program funders require. Results from this survey will assist public and private funders to understand: a) what specific metrics or instruments are meaningful to providers for measuring outcomes, and b) what is the potential for alignment of outcomes measures and other relevant data collection across funders and contract recipients.
Completing the survey: We are asking that a senior administrator from your organization (e.g., clinical director; evaluation lead, executive director) to complete the survey by Wednesday, December 21st, 2016 at 5:00pm CST.
The survey will take approximately 15-25 minutes to complete. Participation is voluntary and the responses will be kept confidential. If you need to exit the survey prior to completion, click on save and return later. Follow the instructions provided to return to the survey.
Thank you to all our children and adolescent service providers who have already completed the survey. If you are a children and adolescent provider and have not yet had a chance to participate, this is a reminder that the survey will close on Wednesday, December 21st, 2016 at 5:00pm CST.
