UNIFORM GRANT AGREEMENT (UGA) FOR FY 2017
If you are going to be funded in FY 2017 by IDHS/DASA, you should have received an e-mail from DHS.DHSOCA@illinois.gov requesting you go to the CSA Tracking System website to download the Uniform Grant Agreement (UGA). The signature page and a completed Exhibit G must be returned on or before July 1, 2016.
Thank you for your patience as we implement grant-making changes for FY 2017. We are prepared for grantees to register in the Grantee Portal and submit their Internal Control Questionnaires. Both of these requirements can be accomplished electronically, and are only needed once for the fiscal year, regardless of how many grants you receive.
IDHS will allow 30 days past the award date to accomplish the pre-award requirements.
To pre-qualify to be a grantee, go to http://www.illinois.gov/sites/GATA/Grantee/Pages/default.aspx and follow the registration process. Please have your DUNS number; SAM CAGE code; verify you are not on the Federal Excluded Parties List; and are in good standing with the Illinois Secretary of State.
Internal Control Questionnaire
After you register, you will receive an e-mail with a link to the Internal Control Questionnaire. Please complete and submit the questionnaire for review.
The Grant Accountability and Transparency Act (GATA) website mentions another requirement. The Programmatic Risk Assessment is needed for each grant you receive and is specific to that program. You can complete the Programmatic Risk Assessment via the IDHS website at a later date. We will send out a separate notice about that process when it is available.
