Team 5 - Monitoring and Quality

The Monitoring and Quality Team will provide recommendations for establishing better collaboration and creating synergy opportunities between PAS Agencies and service providers that lead to better quality and improved health and safety outcomes.

Team Responsibilities

1. Map out strategies for implementing key components of the change initiative.
2. Identify and overcome any obstacles impeding successful implementation of the change initiative.
3. Keep key stakeholders apprised of the progress of the change initiative by way of status reports to the Steering Committee.

Team Focus

1. Oversight and Monitoring of Individual Service Plans and agency progress in fulfilling and achieving individualized goals
2. Reporting requirements
3. Establish new procedures and requirements
4. Establish performance measures to be used by staff who provide Individual Service and Supports Advocacy (ISSA)
