Team 1 - Support Coordination Enhancement

The Support Coordination Enhancement Team will define the role of service coordinators under the new case management system.


Team Responsibilities

  1. Map out strategies for implementing key components of the change initiative.
  2. Identify and overcome obstacles to successful implementation of the change initiative.
  3. Provide Steering Committee with status reports.

 Team Focus

  1. Identify the competencies, knowledge, training and resources needed to perform their jobs competently.
  2. Identify changes needed in structure and/or business processes to support the service coordinators.
  3. Identify a set of performance measures for use in ascertaining the effectiveness of the ISC/PAS agencies.
