Long Term Care Case Consolidation to Macon LTC (163)

Summary:

• Effective March 10, 2014, Long Term Care Cases from Cass (018), Logan (062), Macon (063), Macoupin (064) and Schuyler (092) will be transferred to the Macon County Long Term Care FCRC (163).
•  A copy of the Client Notice announcing the transfer of the Long Term Care cases is attached.

To improve the delivery of services and increase cost effectiveness, the Cass, Logan, Macon, Macoupin and Schuyler Long Term Care cases will be transferred to the Macon LTC Family Community Resource Center (163). A copy of the client notice CN14.10/CN14.10S (pdf) is attached.
