- The Integrated Care Program (ICP) is expanding to four regions in the state. Seniors and persons with disabilities will be required to enroll in the ICP if they receive Medicaid, but are not eligible for Medicare.
- CN 13.18/S, Integrated Care Program Notice (pdf), will be mailed to persons who receive long term care services or home and community based services who are required to enroll in ICP. Persons are getting this notice because they will be sent enrollment material for the Integrated Care Program.
- Persons who receive ICP enrollment material are required to enroll in ICP.
- The enrollment material will give the deadline to enroll. Persons who fail to enroll by the deadline will be automatically enrolled in a health plan.

The Integrated Care Program (ICP) is expanding to allow more persons to enroll as we continue to meet the State's mission to comply with Public Act 96-1501, which requires 50% of Medicaid clients to be enrolled in a form of care coordination by January 2015. The Integrated Care Program (ICP) is expanding to four new regions in the state.

Persons receiving this notice will receive enrollment material for the Integrated Care Program.
