Certified Relationship Development Intervention Consultant

Qualifications: Professional certified to provide Relationship Development Assessment and who has had their credentials approved by the DHS Division of Developmental Disabilities to provide relationship development intervention services under the Illinois Adult, Children's Support and Children's Residential Medicaid Waivers.


How Do I Become A Provider?

  1. You must complete Certified Relationship Development Intervention Consultant Certification Verification: Checklist D Form (IL462-0956) (PDF).
  2. Send completed Checklist D and a copy of your Relationship Development Intervention Consultant certificate to the address provided on the form.
  3. When your credentials are verified and approved by the Division of Developmental Disabilities, the Division will send an approval letter.
  4. Upon receipt of your approval letter, you must complete the enrollment process in one of two ways: 

    1. Independent Provider - Directly Reimbursed 

      If you are a professional who is an independent provider who will bill Department of Human Services (DHS) directly as the provider/vendor, and you will be paid directly by DHS under your own Social Security Number.

    2. Professional - Reimbursed By Other Means 

      If you are a professional who works for an agency, is part of a group practice, limited liability corporation (LLC), or will have another entity or agency acting as a billing agent and the billing agent will be reimbursed by DHS for your services, you must follow the directions for identifying the other agent.
