Immunizations Education Actual Services actual service date, service type code Service type code is IMED. Infant's age in months is calculated for the APORS start date, the quarter ending date and each IMED actual service date. The infant must have an IMED service for each of the following intervals active in APORS: 2 through 5 months, 6 through 9 months, 10 through 15 months, 16 through 21 months, 22 through 25 months.
EPSDT Visits Actual Services actual service date, service type code Service type code is 806. Infant's age in months is calculated for the APORS start date, the quarter ending date and each 806 actual service date. The infant must have an 806 service for each of the following intervals active in APORS: 2 through 5 months, 6 through 9 months, 10 through 15 months, 16 through 21 months, 22 through 25 months.
Face to Face Visits Activity Entry activity date Infant's age in months is calculated for the APORS start date, the quarter ending date and each face to face visit. The infant must have a face to face visit for each of the following intervals active in APORS: 2 through 5 months, 6 through 9 months, 10 through 15 months, 16 through 21 months, 22 through 25 months.
Home Visit

Actual Services

activity date, site of contact, actual service date, service type code Set to 'Yes' if the infant received a home visit between 2 through 4 months old and the quarter ending date. The home assessment and safe sleep education must be completed during the home visit. The services type code is SSED
Developmental Screening Actual Services actual service date, service type code Service type code is 824. Infant's age in months is calculated for the APORS start date, the quarter ending date and each 824 actual service date. The infant must have an 824 service for each of the following intervals active in APORS: 6 through 9 months, 10 through 15 months, 16 through 21 months, 22 through 25 months.
