DHS offers this program for licensed and licensed-exempt home Child Care Providers and Division of Rehabilitation Services Personal Assistants.

Sign-up in 3 Easy Steps

Step 1 - Fill out the form

• Provider Payment Option Form (pdf)
  • Make Sure your current address is listed on the form
  • Mark the DEBIT MasterCard® CARD box

Step 2 - Return the form

• Return the completed & signed Provider Payment Option Form (pdf) to the address on the form.

Step 3 - Activate your Card

• After receiving your card, you must activate your Illinois Debit MaterCard before you can use it.
