Toll-Free Number To Verify Information

Enrolled Medicaid waiver providers may call the Department of Healthcare and Family Services (HFS) Provider Hotline during state working hours to verify an individual's Medicaid enrollment. Up to six inquiries may be made per call. The toll-free number is:

1-800-842-1461.

You will be asked to enter your Medicaid provider identification number. The identification number for providers is the nine-digit Federal Taxpayer Identification Number (FEIN or SSN) followed by a three-digit extension.

  • For the adult waiver, the three-digit extension is usually 100
    For example, 123456789100 (FEIN/SSN + 100)
  • For the children's support waiver, the three-digit extension is usually 400
    For example, 123456789400 (FEIN/SSN + 400)
  • For the children's residential waiver, the three-digit extension is usually 700
    For example, 123456789700 (FEIN/SSN + 700)

To use the automated system, you must have the individual's Medicaid Recipient Identification Number (RIN) and the date of service for which you need eligibility information.

If you do not know the individual's RIN, you need the individual's name, birthdate and SSN and must talk with hotline staff.
