PM 06-21-02: Application Process

Individuals can apply by completing an application and submitting it to the Special Units Office (FCRC 209) in Cook County, to the FCRC serving the applicant's residence outside of Cook County, or by any other means as described in PM 02-04-01.

Eligibility may be backdated up to three months prior to the month of application if the applicant would have met the criteria for the backdated months.

This is a medical only program.
