Becoming a Provider

Within DHS, there are various types of service providers. Select the type of service provider you are interested in and follow the steps to becoming a provider.

Alcoholism & Substance Treatment and Intervention Services Provider

• Substance Use Disorder Treatment and Prevention is the road map to licensing for Alcoholism and Substance Treatment and Intervention Services.
• The Access to Recovery Program accepts applications from licensed treatment, community-based, and faith-based programs that provide support services to clients in recovery from alcohol and/or other drugs.

Developmental Disabilities Provider

• Becoming A New Developmental Disability Provider or an Established Provider Adding New Services

Family & Community Services

• Becoming a Licensed Child Care Provider
• To become an approved Child Care Collaboration Provider call (217) 785-2559 and ask for the Head Start Collaboration Manager.
• Become an Illinois Link Provider and accept SNAP at your retail location.
• You may become a provider by going through the RFP process to secure grant funding. With questions regarding your program/area of interest, contact your Regional Community Support Contact. They will be able to discuss your interest, share information on our various programs and provide you the information needed to become a provider.

Mental Health Provider

• Becoming a Certified Medicaid Community Mental Health Services Provider
  • Organizations interested in becoming certified under 59 Illinois Administrative Code 132 (Rule 132) mus submit documentation of their qualifications to become certified to the DHS Division of Mental Health.

Rehabilitation Services

• Community Rehabilitation Services Providers rules to certify are outlined in Administrative Rule 530 - Criteria for the Evaluation of Programs of Services in Community Rehabilitation Agencies.
• Becoming a New Vocational Rehabilitation Provider
• For Home Services Program Individual Provider enrollment (Non-Agency PA, CNA, LPN or RN):
  • HSP Customers identify and employ their own Individual Providers. If you have been selected by a HSP Customer, please visit the IDHS/HSP Printable Timesheets and Provider Employment Packet page to complete a provider packet for HSP Customer Employment. Employment packets must be submitted to the HSP Customer's local DRS office.
• For Home Services Program Agency Vendor enrollment: Please contact your local DRS/HSP office to enroll as an agency provider

Vendor Services

A Vendor is a business or organization that provides supplies or services to the State for compensation.
