Case Management: Mental Health, Transition Linkage And Aftercare

Services that provide coordination, support and advocacy for consumers who have multiple needs such as mental health, vocational, educational, child welfare and other community services, and require assistance in obtaining them. Assistance in making an effective transition to a living arrangement consistent with the consumer's welfare and development.


Case management can connect you with the services you may need.  This can be especially helpful when you are moving from a hospital or nursing home into the community.  Mental health center staff can help you to find medical, child welfare, employment and other services you might need to live independently.


An evidence-based model of treatment/services that provides an inclusive array of community-based mental health and supportive services for adults (18 years of age and older) with serious and persistent mental illness or co-occurring mental health and medical or alcohol/substance abuse disorders. It requires an intensive integrated package of services, provided by a multi-disciplinary team of professionals over an extended period of time.
