Welcome to the Illinois Child Care Assistance Program! Your participation as a license exempt child care provider makes it possible for Illinois children to enjoy the benefits of quality child care. We appreciate your contribution.
Enclosed is valuable information about the Child Care Assistance Program. You will find the following materials included in this packet:
- Provider rate sheet,
- Information on the Child Care Telephone Billing System (which allows you to receive your payment quicker),
- W-9 Form for you to complete and return, and
- Registration Checklist
All providers who are not licensed by the State of Illinois must register with the Illinois Department of Human Services (IDHS) to receive payment. To register, you will need to send us a copy of your social security card and a copy of the front and
back of your picture ID. The registration checklist is provided to help you remember what you need to give us so you can be paid. Please return these forms within 10 business days.
