WAG 26-01-03: Document Verification Request (Form G-845S)

The G-845S, Document Verification Request, is a turnaround form used to verify an applicant's immigration status. DHS completes the first part of the form and sends it to the United States Citizenship and Immigration Services (USCIS) of the Department of Homeland Security. USCIS researches the applicant's USCIS files and completes the response portion of the form. USCIS checks all appropriate statements on the lower half and back of the form, to indicate the applicant's immigration status and work eligibility. USCIS must return the G-845S and attached copies to the FCRC within 10 workdays.
