Who may participate in the activity:

Include this activity when the person:

• is unable to fully participate in Countable activities due to mental health problems; 
• has been referred to a mental health health provider;
• has been identified by a treatment provider as having a mental health problem;
• has been accepted for mental health treatment.

What the activity requires:  

• Make and keep appointments with mental health provider;
• Cooperate with mental health treatment;
• Verify participation as requested.  

It is not necessary to report time spent in activities.
