PM 20-08-01: Providers

WAG 20-08-01.

Providers may be either private facilities or state-operated facilities. Illinois residents who need care should reside in an Illinois facility. If residence in an Illinois facility is not possible, the client may be eligible for care in an out-of-state facility (excluding SLF). See PM 03-02-00 for residency policy.

Facilities must have a provider agreement with HFS and must be:

• licensed by the Department of Public Health (DPH); or
• certified state-operated facilities, or
• certified by HFS for SLFs.
