Issue benefits in the name and to the address of the guardian unless one of the following situations exists. If the client has 2 guardians, issue benefits to the person designated by the "guardian of the person".
Situation 1: Issue benefits in the client's name and to their address if they have a "limited" or "temporary" guardian, unless the client requests otherwise.
Situation 2: Issue benefits in the name of the guardian, but to the address of the facility, if the client lives in a long-term care facility. But if the guardian wants the MediPlan card, issue benefits in the
guardian's name and to the guardian's address.
