WAG 03-01-04-c: Health Benefits for Immigrant Adults (HBIA)

Verify the following:

1. Applicant is a resident of Illinois.
2. Applicant is aged 42 through 64 yrs. old. Self-attestation is acceptable if the customer's age cannot be verified electronically.
3. Applicant has a noncitizen status that does not meet the criteria for Federal Medicaid. (See PM 03-01-00: Citizen/USCIS (U.S. Citizenship and Immigration Services) Status).
