  1. Program Description
    Public Health AmeriCorps programs will support health equity, mental health and substance use disorders, COVID-19 recovery, and will increase access to services, especially in underserved communities. Public Health AmeriCorps has two overarching goals:
    1. Help meet the public health needs of local communities by providing needed capacity and support in state and local public health settings and advancing more equitable health outcomes for communities who are currently or historically underserved.
    2. Provide pathways to good quality public health-related careers through onsite experience, training, and more, with a focus on recruiting AmeriCorps members who reflect the communities in which they will serve.
  2. SFY25 Continuation Application
    Current Providers will receive a continuation application packet via email. Providers may request additional copies of the continuation application by sending an email to DHS.ServeIllinois.NOFO@illinois.gov
    1. This continuation application is for State Fiscal Year 2025 funding for Public Health AmeriCorps (CSFA #444-80-2970)
      1. Eligibility is limited to current AmeriCorps Formula providers funded under CSFA# 444-80-2970.
      2. Each applicant must be registered in the Illinois GATA Grantee Portal and must continue to be GATA Prequalified. Refer to IDHS: Pre-Qualification Instructions and Resources (state.il.us).
      3. Applicant organizations must complete the FY25 ICQ before an award can be made. (Accessed through the Grantee Portal.)
      4. Applicant organizations must submit a complete FY25 Application Packet by the due date listed in the continuation application to DHS.ServeIllinois.NOFO@illinois.gov. Include "CSFA# 444-80-2970" on the subject line.
    2. A complete application packet includes:
      1. Uniform Application for Grant Assistance
      2. Application Summary Page and Program Narrative UPDATE
      3. Any additional attachments required by your program (listed in the continuation application packet.)
      4. FY25 Budget must be entered into CSA by application due date.
      5. Signed Grantee Conflict of Interest Disclosure.pdf.
      6. Subrecipients: If your organization is proposing to use subrecipients in this program, complete the Subrecipient Section of the continuation application and include corresponding attachments for each subrecipient.
      7. In regard to payment terms, if choosing Advance Payment (Advance and Reconcile) or Working Capital Advance, include the Advance Payment Request Cash Budget IL444-4985.pdf (state.il.us).
    3. The Office of Community and Positive Youth Development reserves the right to request additional information from the applicant to assist in FY25 funding and programmatic decisions.
