Homeless Youth (HY)

  1. Program Description
    The Homeless Youth program serves those youth who are 14 to 23 years of age who cannot return home and/or lack the housing and skills necessary to live independently. The program strives to meet the immediate safety and survival needs of youth (food, clothing, and shelter) and to provide services that help homeless youth transition to independent living and become self-sufficient. Services provided are emergency shelter, outreach and transitional living. These services are available to youth in these programs include housing, food, needed goods, and assistance in obtaining and maintaining available support and services in the community, educational services, basic life skills, employment and/or vocational training. Ensure necessary service referrals to CCBYS, Mental Health, Substance Abuse and Prenatal and Parenting.
  2. FY25 Continuation Application
    Current Providers will receive a continuation application packet via email. Providers may request additional copies of the continuation application by sending an email to DHS.YouthServicesInfo@illinois.gov
    1. This continuation application is for Fiscal Year 2025 funding for Homeless Youth (HY) (CSFA #444-80-0711-01)
      1. Eligibility is limited to current Homeless Youth providers funded under CSFA# 24-444-80-0711-01.
      2. Each applicant must be registered in the Illinois GATA Grantee Portal and must continue to be GATA Pre-Qualified. Refer to IDHS: Pre-Qualification Instructions and Resources (state.il.us).
      3. Applicant organizations must complete the FY25 ICQ before an award can be made. (Accessed through the Grantee Portal.)
      4. Applicant organizations must submit a complete FY25 Application Packet by the due date listed in the continuation application to DHS.YouthServicesInfo@illinois.gov. Include "CSFA# 25-444-80-0711" on the subject line.
    2. A complete application packet includes:
      1. Uniform Application for Grant Assistance (pdf)
      2. Application Summary Page and Program Narrative UPDATE
      3. Any additional attachments required by your program (listed in the continuation application packet.)
      4. FY25 Budget must be entered into CSA by application due date.
      5. Signed Grantee Conflict of Interest Disclosure.
      6. Subrecipients: If your organization is proposing to use subrecipients in this program, complete the Subrecipient Section of the continuation application and include corresponding attachments for each subrecipient.
      7. In regard to payment terms, if choosing Advance Payment (Advance and Reconcile) or Working Capital Advance, include the Advance Payment Request Cash Budget IL444-4985.pdf (state.il.us)
    3. The Office of Community and Positive Youth Development reserves the right to request additional information from the applicant to assist in FY25 funding and programmatic decisions.
