DDD Communication - Updated EVV Live-In Caregiver Attestation Forms and Process

On July 20, 2023, the Division of Developmental Disabilities (DDD) announced the posting of an Informational Bulletin outlining the process to request the Electronic Visit Verification (EVV) Live-In Caregiver Exemption.

The Information Bulletin includes links to the original Live-In Caregiver Attestation Forms (English version and Spanish version), and other information related to the Live-In Caregiver Exemption process. The 07/20/2023 DDD Communication also notified the stakeholder community DDD was in the process of developing a revised version of the Live-In Caregiver Attestation Forms. The revised forms have been finalized and posted to the DDD website at the links previously provided.
Personal Support Workers (PSWs) who are requesting the live-in caregiver exemption are, effective immediately, required to use/complete the revised forms for requesting the exemption. If you already submitted the original form to request your exemption, it will be honored until you are required to submit the form again. The next time you need to submit the form, whether it is for the annual submission or if something changes over the course of the year that affects your exemption status, you will need to use the revised form.
On August 2, 2023 DDD held and recorded a Live-In Caregiver Exemption Process and Form Training on August 2, 2023. The training recording is available on the DDD EVV webpage: IDHS: Illinois' Electronic Visit Verification (EVV) (state.il.us). During the training, DDD discussed what is required for each section of the Live-In Caregiver Attestation Form and the supporting documentation requirements. DDD also discussed how you should submit your packet, Independent Service Coordination (ISC) agency responsibilities for verifying the living arrangement, and any potential liabilities for submitting inaccurate documentation.
Once the Attestation Form is completed, signed by all required parties, and supporting documentation is gathered, it must be submitted to ACES$ or the PSW Agency serving the individual. If you purchase PSW services through a PSW Agency, you will want to contact the agency to determine what methods they will use to collect the information. If you purchase services through ACES$, you can submit your packet via the following methods:

As a reminder, if the PSW is not requesting the live-in caregiver exemption, there is no need to submit the Live-In Caregiver Attestation Form. As for those PSWs requesting the exemption, the Attestation Form must include all information in order to be accepted/approved. The Attestation Form must contain electronic or original signatures from the PSW and the Employer of Record (EOR). Writing the PSW or the EOR's name in italics in this space is not an acceptable signature. Additionally, the Attestation Form must be accompanied by the supporting documentation proving the PSW and individual's addresses. If there is a document listed as approved supporting documentation but it does not include the individual's name and address, it will not be accepted. The only acceptable forms of supporting documentation are listed in the DDD Information Bulletin (IDHS: Live in Caregiver Exemption (state.il.us)).
If the individual receives services through ACES$, all PSWs, EORs, and SDAs are encouraged to register via ACES$ Online. PSWs may download CareAttend, the ACES$ EVV application, to a smart device at any time. To register for upcoming ACES$ trainings, please go to: ACES$ Online. All are encouraged to attend regardless of Live-In Exemption status. If the individual purchases PSW services through an Agency-Based PSW provider, the PSW should be receiving training from their PSW Agency in August 2023. Please reach out to your PSW agency for information on those trainings.
