GA-Illinois – Equitable Access Towards Sustainable Systems (EATS) funded by LFPA(24-444-80-3189-01)

To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire and the Grant application.

At the time of the application, all applicants must submit the following documents:

• Uniform Application for State Grant Assistance (pdf))
• CSA Budget/Tracking System and Registration Information

Complete applications must be received by 10/31/2023 12:00pm CST

The completed documents are to be emailed to the Bureau email address DHS.OFWContracting@illinois.gov or Melanie.M.Kluzek@illinois.gov with the subject line stating:

• Last 4 digits of CSFA #3189
• Provider Organization Name- for example, 3189 Jane's Agency or 3189 HOPE Agency

Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
