(FCRC) Request a copy of the trust agreement;

Complete form HFS 2150 and email the form and the trust document to HFS OIG LTC-ADI at HFS.OIG.LTC-ADI-Legal@illinois.gov or fax to 217-524-3030.

Notifies the FCRC of:

• whether the trust is revocable, irrevocable or exempt;
• if resources held in the trust are countable; and
• if the terms of the trust permit income disbursements.

Upload HFS OIG analysis to the case.

For NH, SLP or DoA HCBS (AABD, ACA)

(LTC Hub)

• complete and upload form HFS 3654A;
• make sure all verifications are uploaded to the case;
• complete OIG referral fields in IES.

Uploads LTC-ADI Directive and resource verifications to the case.
