To be eligible for a contract, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, and Grant Application. The Grant Application is to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.
All potential grantees must contact their respective lead Continuums of Care in order to complete and submit the required Grant Funding Packet. The lead Continuums of Care have jurisdiction in recommending qualified grantees to the State of Illinois, and in determining how to allocate ESG funds each program year.
At the time of application, all Bureau grantees must submit for each grant program the following documents:
The completed documents are to be emailed by May 31, 2023 to the Bureau email address DHS.ESG@Illinois.gov and copy your assigned Grant Manager with the subject line stating:
- Last 4 digits of CSFA #0496
- Provider Organization Name - For example, 0496 Jane's DV Agency or 0496 HOPE Agency
Note: Everything is being tracked by the CSFA number, so it is important that all required materials and correspondence contain this information.
