Dear Stakeholders:
As a reminder, earlier this year the Division of Developmental Disabilities (DDD) engaged the University of Illinois Chicago (UIC) to evaluate and strengthen our Person-Centered Planning (PCP) process. In May, the DDD shared UIC's final report which can be found online. Since its release, the DDD has worked to implement several of the recommendations including updating the PCP forms. The DDD has received the final versions of the PCP forms and is now moving forward with implementation.
Beginning October 1, 2022, all Discovery meetings should be completed using the new version of the Discovery Tool, followed by the updated Personal Plan and finally the newly developed Implementation Strategies. These forms are now available in a fillable PDF version, all attached to this email. The PCP webpage is currently being updated to include links to each of the forms as well as a separate FAQ page. As soon as the webpage is updated, the DDD will communicate with stakeholders.
Additionally, the DDD has published a Draft Information Bulletin on the PCP process. Per the usual process, public comments can be submitted on the Information Bulletin for the next two weeks (until September 28, 2022). The Draft Information Bulletins webpage and the PCP process Draft Information Bulletin have all the informationi. The PCP webpage will also include the link to the Draft Information Bulletin.
The DDD understands that previously, agencies may have developed their own template for the Implementation Strategies. At this time, the DDD is now requiring all agencies to use the DDD form. If your agency uses an electronic record system and would like to incorporate the new form into your existing software, please reach out to Kristin Semanik (Kristin.Semanik@illinois.gov) for approval of your template.
The DDD previously hosted 4 trainings to review the changes to the PCP process. The recordings of the trainings can be found here. The DDD will also offer ongoing training on the PCP process with the next training on October 27, from 1pm - 2pm.
