Send the client a Reconciliation Appointment Notice (Form 2846) to schedule an appointment.

Notify the mental health liaison of the date of the reconciliation meeting if the issue involves a mental health-related issue.

Give or send the client another Form 2846 if a meeting with the mediator must be scheduled.

Use Reconciliation Agreement (Form 2846A) to record the action the client must take to show compliance with the requirement and the timeframe for the action, if the action must be taken after the meeting. The timeframe cannot be longer than 2 weeks. All persons taking part in the reconciliation process are to sign the form: client, worker, and mediator, if any.

Give the client the Form 2846A. File a copy in the record. Provide any other information or instructions the client may need to complete the agreement. Set a control to follow up at the appropriate time.

When the client is able to show cooperation at the meeting and no additional action is needed, do not complete Form 2846A. Write up the meeting in the client's record.
