PERSON-CENTERED PLANNING TEMPLATES RELEASED
Earlier this year, the Division of Developmental Disabilities engaged UIC to collect feedback, survey stakeholders and identify areas where we could strengthen the person-centered planning process. The result is An Evaluation of Person-Centered Planning in Illinois Adult DD Waiver Services.
Attached are updated templates developed in partnership with UIC and stakeholders, including our Independent Service Coordination agencies. The Division will be requiring use of these updated templates starting July 1, 2022. The templates include an updated Discovery Tool, Personal Plan and a new, Division required, Implementation Strategy. Each of the documents now include instructions.
In the upcoming week, the templates will be posted on the DDD website. In addition, DDD will also finalize updated fillable forms for the templates.
Please remember to attend the final three PCP webinars that will go into more depth on each of the templates.
