WAG 03-11-01-e: Clients with Multiple SSNs

Some persons have been issued more than one SSN. If there is reason to believe that a person has more than one SSN, take the following action:

  1. (FCRC) Obtain copies of:
    • the person's birth certificate, and
    • any SSN cards or other documents the client has that show an SSN.
  2. (FCRC) Complete a Form 1925, and attach a copy of the birth certificate, and a copy of any SSN cards or other documents.
  3. (FCRC) Send the completed Form 1925 and attached copies to the SSA PCU.New Manual Textor email to dhs.ssapc@Illinois.gov.
  4. (SSA PCU) Investigates the discrepancy to determine the correct SSN. If unable to resolve, contacts SSA.
  5. (SSA) Designates one SSN to be used by the client, and notifies SSA PCU of the designated number.

    NOTE: Since all SSNs assigned to a person remain valid, SSA provides SSA PCU with any other SSNs assigned to the client. 

  6. (SSA PCU) Enters the SSN designated by SSA Revised manual textin IES on the Individual Information page, checks the Validated by SSA PC box and updates the Verification Source to Verified by SSA/SOLQ.
  7. (SSA PCU) Sends the Family Community Resource Center a memo identifying the designated SSN, and any other SSNs provided by SSA.
