WAG 03-11-01-c: SSN Requirements at Application (Cash and Medical)

If a person provides an SSN:

  1. Verify the SSN by viewing the applicant's Social Security card, SOLQ,New manual text FDSH clearances, or a document such as a driver's license or other card with their signature or photo.
  2. Enter the SSN in IES on the Individual Information page.

If a person does not have an SSN:

  1. Tell the person that an application for an SSN is a condition of eligibility (except for GA, Moms and Babies, babies born to Medicaid eligible mothers, and noncitizen children who have certain nonimmigrant visas or are undocumented).
  2. Complete a Requirement to Apply for Social Security Number Card (Form 3168). Issue one copy to the client and upload a copy in IES to the ECR.
  3. Give the person Form 3168 and Form 267 or Form 1721 and tell them to report to the local Social Security District office. Tell the client they must give SSA proof of age, identity, and citizen/INS status.
  4. Allow the client 10 calendar days to return proof that an application for an SSN has been filed.

New Manual TextIf it is unknown an individual has an SSN or unknown that the individual is willing to apply for an SSN, complete the SSN Application Verification section of the Individual Demographics-SSN Application/Armed Services page in IES with the following information:

  • Does Individual have an SSN? select No
  • If no SSN, has an SSN been applied for? select No/Unknown
  • Is individual willing to apply for an SSN? select No/Unknown
  • Reason unwilling/unable to apply for SSN? select applicable reason:
    • Children 6 months or younger (EDGs do not pend/deny)
    • Circumstances beyond the clients control prevent them from securing proof required to obtain an SSN (SNAP only)
    • Not a valid Reason (EDGs deny)
    • Not applicable (FHP)
    • Other (EDGs do not pend/deny)
    • Undocumented alien not required to apply (EDGs do not pend/deny)

If an individual does not have but has applied for an SSN complete the SSN Application Verification section of the Individual Demographics-SSN Application/Armed Services page in IES with the following information:

  • Does Individual have an SSN? select No
  • If no SSN, has an SSN been applied for? select Yes
  • SSN Application Verification: Select from one of the following:
    • SS-5 Application for a Social Security Card (EDGs do not pend)
    • SN-5028 Receipt for application for a social security number (EDGs do not pend)
    • SSA-2853 Information about when you will receive your baby's social security card (EDGs do not pend)
    • Modified birth document that includes the minimum required information (EDGs do not pend)
    • SOLQ (EDGs do not pend)
    • Client statement (EDGs pend)
    • Not verified (EDGs pend)

If an individual does not have a SSN, has not applied for an SSN, but is willing to apply for an SSN, complete the SSN Application Verification section of the Individual Demographics-SSN Application/Armed Services page in IES with the following information:

  • Does Individual have an SSN? select No
  • If no SSN, has an SSN been applied for?  select No/Unknown
  • Is individual willing to apply for an SSN? select Yes
  • SSN Application Verification: Select the verification provided

If an individual refuses to comply with SSN requirement and is unwilling to provide their SSN complete the SSN Application Verification section of the Individual Demographics-SSN Application/Armed Services page in IES with the following information:

  • Does Individual have an SSN? select Yes
  • Reason unwilling/unable to apply for SSN? select appropriate reason. Select No Valid Reason if an individual refuses to provide the SSN (EDGs will deny)

NOTE: For cash benefits, do not process the application as "presumptively eligible" (PE) pending the receipt of an SSN. Applying for an SSN meets the SSN requirement.
