PM 06-11-02-c: Community Habilitation Services

WAG 06-11-02-c.

Clients living in the community who receive only habilitation services through DHS are considered receiving Community Habilitation Services. These services are provided as part of the In-home Care Program (see PM 20-28-00). These services are designed to promote independence in daily living and integration into the community.

In order to receive Community Habilitation services, a client must:

  • be developmentally disabled,
  • be age 18 or over, and
  • require only habilitation services as determined by DHS - Office of Developmental Disabilities (ODD).
