The Illinois Department of Human Services Act gives the DHS Office of Inspector General (OIG) statutory responsibility to conduct investigations into allegations of abuse and neglect at DHS Developmental Centers and Mental Health Centers; as well as community agencies funded, licensed or certified by DHS to provide mental health or developmental disability services. OIG's investigative processes are codified in Title 59, Chapter 1, Part 50 of the Illinois administrative Code.

Data Elements and Definitions
Substantiated - There is a preponderance of the evidence to verify the substance of the allegation.
Unsubstantiated -There is credible evidence, but less than a preponderance of evidence to verify the substance of the allegation.
Unfounded - There is no credible evidence to verify the substance of the allegation.
Recommendation - An admonition rendered by OIG, separate from a finding, that requires action by the facility, agency or Department to correct a systemic issue, problem or deficiency identified during an investigation which is documented on a Written Response.
