Self-Management Goal-Setting Tools in Integrated Behavioral Health

This lecture and discussion-based presentation will share the quality improvement project of the self-management goal-setting procedures at a local integrated behavioral health primary care clinic. The project background, methods, results, and practice implications will be explored. To implement knowledge translation, the final Self-Management Goal-Setting Tools created for the clinic will be illustrated for future dissemination at further sites to advance evidence-based practice, quality care, and integrated behavioral health. Persons with any knowledge/interest in mental health or lived experience as well as administrators may attend.

Amanda Hopcroft, OTD, OTR/L is an occupational therapist from Chicago, IL with particular interests in mental health, integrated care, neurorehabilitation, self-management, health equity, and education. Amanda received her post-professional Doctorate in Occupational Therapy degree after having completed a quality improvement project of the self-management goal-setting procedures at an integrated behavioral health primary care site where no occupational therapists had been on staff. Amanda has unique experience as a Clinical Instructor at the University of Illinois at Chicago teaching an undergraduate occupational therapy course titled Promoting Wellbeing, which aims to enhance equitable student mental health and academic success through the development of supportive skills that promote self-reflection and overall wellbeing.
