Annual Certification Plan (ACP) is Live
Organizations that receive contract funding from the Illinois Department of Human Services/Division of Substance Use Prevention and Recovery (IDHS/SUPR) are required to submit an Annual Certification Plan (ACP).
The SFY2021 ACP, when certified by the organization and accepted by IDHS/SUPR, formalizes service and funding expectations contracted between IDHS/SUPR and the organization.
Annual Certification Plans and all related forms are submitted electronically through the following link: https://www.surveymonkey.com/r/ACPFY21.
Please note that the submission must be complete with all questions answered and include all related forms.
The FY21 ACP is due no later than February 15, 2021. We ask that you please give this your immediate attention.
