Verifications are requested from the customer after the interview has been conducted. If at the interview it is determined that verifications are needed to establish eligibility, the customer will be issued a Verification Checklist indicating the required verifications that must be provided and the due date. The customer is allowed 10 calendar days to return the requested verification(s). The application is denied on the workday following the last day to provide the verification, if the customer has required verification(s) but does not provide it by the due date. The applicant is not allowed more time to provide the verification(s).
Interviews are not conducted for medical programs. If medical is the only program request and additional information is needed, a Verification Checklist is issued to the customer requesting the information needed to process the medical only application. If the customer does not respond or there is an inadequate response, the application is denied.
