WAG 02-05-08: Procedures for Persons Released from DHS Facilities

  1. (DHS Facility) Sends Form 2378B, the Verification Checklist for Resident of DHS Facility (Form 2849), and eligibility verifications, if any, to apply for residents about to be discharged to the community.
  2. (Screener) Complete the mail-in screening process through AIS. The application date is the date of receipt.
  3. (Screener) Complete and mail Form 267, Form 267-TPL, Form 267D, and pamphlets to the facility.
  4. (EW) If facility staff contact the Family Community Resource Center to advise of a change in the date of discharge and the date is too late to meet required processing timeframes, advise facility staff that the application will be denied. The facility may submit a new application prior to the new anticipated date of discharge.
  5. (EW) If all required information is obtained prior to discharge, determine eligibility as soon as possible.
  6. (EW) Establish a control to approve the case no later than the first workday following the date of discharge. Approve the application no later than the first workday following the date the facility verifies the discharge.
    1. If facility staff contact the Family Community Resource Center to report a change in address where the resident is going upon discharge, continue processing the application. If the application is approved, immediately transfer the case to the correct local office.
