    If you live in a nursing facility in Cook County or a specialized mental health rehabilitation facility (SMHRF) anywhere in Illinois, or you know someone who does, and you are interested in learning more about living in the community and the support services that may be available, the Comprehensive Class Member Transition Program may be able to help you.

    Please call our hotline for referrals and inquiries at (312) 793-7205. Someone from the Comprehensive Class Member Transition Program will talk to you about your situation. If you are eligible, they will connect you to a community agency, called a Prime Agency, that serves the facility where you live. This agency will explain the program in more detail. If you are interested, they will conduct an assessment to see what services and supports you or the person you are referring might need to live in the community. If you or the person you are referring are recommended to move out, the Prime Agency will help you find housing and move out of the facility. After moving out, the Prime Agency will provide supportive services and follow-up to ensure that you are safe and healthy.

    Class Members who participate in the Comprehensive Class Member Transition Program keep all their rights and services. Staff at the facility where you live cannot take any actions against you or treat you badly if you want to move to the community. This is called retaliation and is against the law. Some types of retaliation include neglect, hostility, not supporting you, restricting your privileges, or creating barriers to communication. If you feel you are experiencing retaliation, discrimination, or other similar actions, you can call the Retaliation Hotline at (833) 515-0846. You can also report retaliation to your facility's assigned Ombudsman or the Illinois Department of Public Health Nursing Home Hotline (1-800-252-4343).

    If you live in a nursing facility outside of Cook County, you may be able to get support through the Department of Healthcare and Family Services. You can submit a referral form through Healthcare and Familly Services (HFS) website.
