WAG 01-01-01-a: Complaints of Alleged Discrimination

  1. (FCRC) Give Client/Applicant Discrimination Complaint Form (Form 4026) to anyone who alleges discrimination (complainant).
  2. (Complainant, All Programs) Sends written complaint to one of the following:

  6. (LOA) Take the following action if a written complaint of alleged discrimination is received by the Family Community Resource Center:
    • Send complaint to the Equal Employment Opportunity (EEO) Office, Chicago or Springfield.
    • Send any follow-up correspondence relating to the complaint to the EEO Office.
  7. (EEO Coordinator) Investigates complaint when completed discrimination form is received.
  8. (EEO Coordinator) Submits findings to Chief EEO Officer.
  9. (Chief EEO Officer) Reviews findings.
    1. Directs EEO Coordinator to notify complainant whether or not the case has merit.
    2. Seeks more information or counsels the concerned parties, when necessary.
    3. Notifies LOA or Director of current status of the complaint.
    4. Recommends a course of action to be taken by Department.
