  • Change of WIC Vendor Information Notice (pdf)

    • This form is to be used to notify the office of any WIC Vendor changes in status, location, or formula supplier

  • WIC Incident Reporting Form (pdf)

    • This form is to be completed and submitted to DHS to report any incidents or complaints against a vendor or a participant. All complaints are reviewed and processed accordingly.

  • WIC Vendor Withdrawal Form (pdf)

    • This form must be completed and submitted by any vendor wishing to end their authorization as a WIC vendor or in the case of the sale or closing of a business.
