B. Homeless Youth (HY) - Logic Model

GOAL:  Youth are safe and stable.

ELIGIBILITY REQUIREMENTS:  Youth 14-23 years old that lack safe and stable housing

ACTIVITIES

  • Assessment (emergency/safety, Ansell-Casey)
  • Trauma Informed Services 
  • Increase number of contacts with homeless youth
  • Emergency family negotiations
  • Family communication
  • Family counseling
  • Develop and ensure Stabilized housing options
  • Create opportunities for youth to interact with peers and community partners.
  • Access and Provide Life skills education
  • Transportation planning
  • Provide volunteer and Service-Learning opportunities
  • Employment Services:  work readiness; job seeking skills; job maintenance skills & support, etc.
  • Education Advocacy & Support; GED enrollment/support; vocational training; college readiness, etc.
  • Teach parenting skills
  • Prenatal care
  • Pregnancy prevention
  • Develop/provide access to child care
  • Develop a community awareness/cultural competency/sensitivity plan
  • Foster community engagement
  • Connect families to DHS Local Area Offices; ABE website, etc.

STRATEGIES

  • Create/ensure safe environments
  • Conduct Emergency/Safety screens
  • Emergency Care Planning and Implementation - including basic survival /safety needs (health, food, clothing, shelter etc.)
  • Provide Emergency Shelter / Interim Housing
  • Provide Transitional Living Programs
  • Provide Outreach Activities
  • Drop-in centers
  • Develop and Implement individualized case plans based on appropriate assessments
  • Engage and include family members
  • Ensure youth receive educational support & advocacy services
  • Ensure youth receive employment support, development, training, and placement services
  • Increase skills of youth to access needed services within the community
  • Increase basic life skills of youth
  • Connect youth to eligible mainstream benefits (SSI, Medicaid, TANF, SNAP, WIC etc.)
  • Ensure necessary Service Referrals e.g.: CCBYS, Mental Health, Substance Abuse and Prenatal etc.
  • Develop and Implement Discharge Plan

INTERMEDIATE OUTCOMES

  • Youth will experience increased safety
  • Youth will have increased Life Skills
  • Youth will be employed
  • Youth will be actively enrolled in educational programming (high school/ GED/vocational training etc.)
  • Youth will have a HS Diploma or GED
  • Youth will be in stable living arrangements
  • Youth will be connected to mainstream benefits

OUTCOMES

  • Increase the number of self-sufficient youth among the homeless youth population
  • Fewer homeless youth
