FY19 DMH OPEN COMPETITIVE PROGRAM
One new Division of Mental Health Notice of Funding Opportunities (NOFO) for State FY 2019 has been re-posted on the DMH Webpage at https://www.dhs.state.il.us/page.aspx?item=101669 for program 730, Quality Administrator.
For program 730, Quality Administrator, applicants must follow all instructions listed in the NOFO.
Section C, #1 and #2 "Eligibility Information" criteria of the NOFO has been revised for Williams and Colbert Consent Decrees.
Williams Consent Decree: Applicant must be a Medicaid Certified community mental health center, with Assertive Community Treatment or Community Support Team services and covers the identified community areas of Evanston, Uptown, Rogers Park, North east side Chicago, Edgewater and Ravenswood, with capacity to provide strength-based, person-centered engagement to Class Members and coordinate and facilitate a seamless community transition from one of the 24 Specialized Community Mental Health Rehabilitation Centers located in Cook County. Additionally, applicant must be eligible to apply for and receive Transition Coordination funding award.
Colbert Consent Decree: Applicant must be a Medicaid Certified community mental health center that covers the identified community areas: including of the west and northwest side of Chicago and the near West and near Northwest suburbs such as Norridge, Melrose Park, Franklin Park, Schiller Park, Harwood Heights, Bellwood and Maywood; Evanston, Uptown, Rogers Park, North east side Chicago, Edgewater and Ravenswood, with capacity to provide strength-based, person-centered engagement with Class Members to coordinate and facilitate a seamless community transition from Skilled Nursing Facilities/Long Term Care facilities located in Cook County. Additionally, applicant must be eligible to apply for and receive Transition Coordination funding award.
The effective dates to apply are 4/30/2018 through 5/4/2018. Deadline is 5:00 pm CST.
