Person Centered Planning Process For Medicaid Waiver Services

Through the Life Choices System Transformation efforts and in accordance with Federal Home-and Community-Based Services Medicaid requirements, the Division of Developmental Disabilities is implementing a new person-centered planning process for community-based Medicaid Waiver services effective July 1, 2017. To aid in this implementation, we have created this web page to share important documents and information with consumers, providers, independent service coordinators, and other interested parties.

Individuals served in community-based services (e.g., Community Integrated Living Arrangements (CILA), Day Programs, Home-Based Services and Supports (HBS)) are required, under federal regulations, to have plans that are updated at least annually. During Fiscal Year 2018 (July 1, 2017 through June 30, 2018), as individuals' plans come due for update, the State will be transitioning to this new planning process.

We appreciate all stakeholders' cooperation and patience as we move through this transition year. Please see the documents below for general information and use. We will update this web page as new information and materials become available.
