GA-0733 SYT-I - Substance Abuse Treatment for Youth in Transition Initiative

To apply for a SFY2018 DHS DASA award for the same services you were providing in SFY2017, all grantees must complete the Pre-Qualification Process, Internal Controls Questionnaire, Programmatic Risk Assessment and Grant application. The Grant Application and Programmatic Risk Assessment are to be completed for every program/grant. See Grant Application Information and Instructions webpage for more detail.

At the time of application, all grantees must submit for each grant program the following documents:

  1. Grant Application GA-0733
  2. Uniform Grant Budget printed from the CSA Tracking System.

Budgets must be entered into the CSA System.

Submit the completed grant proposal to DHS.GrantApp@Illinois.Gov and submit the proposal electronically on or before May 12, 2017 at 12:00 p.m. Applications must be emailed.

The Agency Opportunity Number and the program contact must be in the subject line.

Specifically, the subject line must be:
