Scope of Service

The Provider will continue with the implementation of the 'First.IL' model of First Episode Psychosis (FEP) Treatment consistent with the training and technical assistance provided by the 'BeST Center' in Ohio utilizing evidence-based services for individuals experiencing a First Psychosis Episode.

Deliverables

The FEP Coordinated Specialty Care Team will:

  1. Establish a Coordinated Specialty Care (CSC) Team in each agency that at a minimum includes Prescriber, Team Leader, Individual Resiliency Training (IRT) Therapist, Individual Placement and Support (IPS), Supported Employment/Supported Education, and Case Manager
  2. Implement outreach and community-based education on the symptoms of Schizophrenia Spectrum Disorders.
  3. Utilize marketing tools consistent with the Division of Mental Health (DMH) marketing portfolio provided, including 'FIRST.IL' Brochures, Hospital Cards and Flyers.
  4. Conduct initial engagement and assessment of persons referred using both site-based and outreach methods to assure the timely completion of initial mental health assessments for individuals referred.
  5. Serve individuals identified as experiencing a FEP with intent to decrease hospitalizations and begin a pathway to recovery, regardless of Medicaid eligibility status, Medicaid, Medicaid Managed Care assignment, or private insurance coverage limits.
  6. Integrate the early intervention, shared decision making, and CSC Team model features of the 'FIRST.IL' model into their organization.
  7. Participate in FEP trainings, learning collaborative teleconferences, and project meetings conducted by DHS/DMH.
  8. Develop plan for sustainment

One provider will act in a fiduciary capacity and provide the following services:

  1. Payment for additional FEPP training and conferences
  2. All other fiduciary expenses agreed upon between DMH and Provider

Reporting Requirements:

  1. Financial Report in accordance with Exhibit C.
  2. Performance Report in accordance with Exhibit E.

Payment Terms

Payment will be issued monthly and reconciled on the basis of reported allowable expenses per the Grant Funds Recovery Act [30ILCS 705/7 and 8].

The Provider shall report quarterly allowable grant expenses on the appropriate DMH reporting template to the DMH program contact no later than November 1, February 1, May 1, and August 1, and reported expenses should be consistent with the submitted annual grant budget. If any budget variances are noted, the DMH program contact may request that the provider submit a revised grant budget before subsequent monthly payments will be made. Performance Measures

The Provider shall report monthly performance on the appropriate DMH reporting template to the DMH program contact no later than the 15th day of the month following the reporting period. DMH program contacts and reporting templates can be found in the Provider section of the DHS website.

The following are included in the reporting template:

  1. Coordinated Specialty Care (CSC) Team including but not limited to Prescriber, Team Leader, Individual Resiliency Training (IRT) Therapist, Individual Placement and Support (IPS), Supported Employment/Supported Education, and Case Manager
  2. Provide outreach and community-based education on the symptoms of Schizophrenia Spectrum Disorders
  3. Number of clients reached through the use of marketing tools
  4. Number of referred client's initial engagements completed within 48 hours
  5. Number of individuals served experiencing First Episode Psychosis (FEP)
  6. Evidence that the early intervention, shared decision making, and CSC Team model features of the 'FIRST.IL' have been integrated into the organization
  7. Number of FEP trainings, learning collaborative teleconferences, and project meetings conducted by DHS/DMH
  8. Have a sustainability plan in place

Fiduciary Measures:

  1. Number of FEPP conferences and trainings
  2. Amount of other fiduciary expenses

Performance Standards

  1. Coordinated Specialty Care (CSC) Team will be staffed with Prescriber, Team Leader, Individual Resiliency Training (IRT) Therapist, Individual Placement and Support (IPS), Supported Employment/Supported Education, and Case Manager
  2. Provided outreach and community based education on the symptoms of Schizophrenia Spectrum Disorders
  3. 2 or more FEP Clients will be reached through the use of marketing tools
  4. 100% of referred client's initial engagements completed within 48 hours
  5. 5 or more individuals served experiencing First Episode Psychosis (FEP)
  6. Integration of early intervention, shared decision making, and CSC Team model features of the 'FIRST.IL' have been completed.
  7. Coordinated Specialty Care (CSC) Team will attend 95% of FEP trainings, learning collaborative teleconferences, and project meetings conducted by DHS/DMH
  8. A sustainability plan is in place and can be submitted upon request

Fiduciary Standards:

  1. 100% of additional FEPP training and conferences paid in full
  2. 100% of other fiduciary expenses paid
