0803 Housing Money Follows the Person

Scope of Service

The Provider will transition targeted Money Follows the Person (MFP) eligible individuals diagnosed with serious mental illnesses from non-IMD (Institution for Mental Disease) nursing homes to integrated Housing community based settings, using criteria of eligibility, clinical assessment and determination of appropriateness and functional capability to successfully move to community living with support services.

Deliverables

The Provider will:

  1. Utilize clinical information provided from non-IMD sources and Transition Coordination assessment tools for appropriately transitioning into a community based living arrangement of their own unit/apartment or a unit/apartment shared with up to four unrelated adults.
  2. Coordinate medical and psychiatric transfer of needs and monitor effectiveness of services, housing, stability and outcomes after community transition.
  3. Provide, on an on-going basis, the appropriate level of community support services for the MFP consumers to maintain themselves in the community and comply with the MFP Program service model.

Reporting Requirements:

  1. Financial Report in accordance with Exhibit C.
  2. Performance Report in accordance with Exhibit E.

Payment Terms

Payment will be issued monthly and reconciled on the basis of reported allowable expenses per the Grant Funds Recovery Act [30ILCS 705/7 and 8].

The Provider shall report quarterly allowable grant expenses on the appropriate DMH reporting template to the DMH program contact no later than November 1, February 1, May 1, and August 1, and reported expenses should be consistent with the submitted annual grant budget. If any budget variances are noted, the DMH program contact may request that the provider submit a revised grant budget before subsequent monthly payments will be made. DMH program contacts and reporting templates can be found in the Provider section of the DHS website. In addition, client detail should be included on the Financial Report.

Performance Measures

The Provider shall report quarterly performance on the appropriate DMH reporting template to the DMH program contact no later than November 1, February 1, May 1, and August 1. DMH program contacts and reporting templates can be found in the Provider section of the DHS website.

The following are included in the reporting template:

  1. Percentage of referred Resident Reviews (determined by outside resident reviewer to be eligible for MFP and appropriate for transition) that were enrolled in MFP for initiation of transition.
  2. Percentage of initiated Transitions that result in community placement
  3. Percentage of Transitions that resulted in sustained community placement for a minimum of three months
  4. Percentage of Transitions that resulted in a sustained community placement for one year.

Performance Standards

  1. 80% of referred Resident Reviews (determined by outside resident reviewer to be eligible for MFP and appropriate for transition) will be enrolled in MFP and initiate transition.
  2. 90% of initiated Transitions will result in community placement (barring unusual circumstances)
  3. 85% of Transitions will result in sustained community placement for a minimum of three months (barring unusual circumstances)
  4. 60% of Transitions will result in a sustained community placement for one year.
