COMMUNITY MENTAL HEALTH NEEDS ASSESSMENT SURVEY

The IDHS/Division of Mental Health requests your input. Please take time to complete the Needs Assessment Survey.

Who should complete the survey?

Persons being treated for mental or substance use disorders; persons in recovery from mental or substance use disorders; and, persons with lived experience of substance use or mental health disorders.

What is it?

This survey is brief and consists of nine questions to obtain your perception of consumer leadership, gaps in service and barriers to receiving recovery-oriented services in Illinois. A few demographic questions are also included to ensure we have received input from the diverse populations of our state. You are free to skip any questions you are not comfortable responding to.

Why should I submit one?

Your voice matters! The State of Illinois will use information gathered from the Needs Assessment Survey to guide our work in developing a more recovery-oriented system of care.

When do I have to have it done?

Responses will be accepted until May 20th.
